Most Frequently Asked Questions  
  1. Does one of the recommendation forms have to be completed by a Dean of the medical school?
    In this case, the Dean is just a suggestion or recommendation from the Board. You can have the recommendation forms completed by any two physicians who have supervised or worked with your in recent months.
     
  2. Do fingerprints need to be done by a professional?
    These fingerprints need to be done by someone at your local police station or sheriff’s office or there could possibly be someone in the Security Office at the institution you are now in that could do them. Your license will be held up if the quality of the prints are not good and get rejected. So do not do them yourself.  Digital prints are not accepted.
     
  3. Where does the appointment letter come from that must be submitted with the license application?
    The appointment letter comes from the House Staff Office. That letter will be included with your application prior to forwarding to the Board for your license to be issued. You do not have to do anything to obtain this letter.
     
  4. Can I send in the license application minus the medical school certification form?
    Yes. If your medical school will not complete the form until you have actually graduated, you can go ahead and send in the rest of the application and we can review all but that one form. If this information is all correct, the license will just be pending receipt of that one form.
     
  5. What is needed to change my name if I am recently married and I want all my residency information as well as license to have my married name?
    You must send in a copy of your marriage license with your license application to the House Staff Office in order for the Board to issue your license in your married name. Our office will also use that marriage license to change your name in all the various areas in the hospital.
     
  6. Do I need to complete the AMA and Federation Request Forms in the license application packet?
    These forms are not required for incoming residents who are just graduating from medical school. If you have been out of medical school for 6 months or more or graduated from a foreign medical school, you must complete these forms.
     
  7. In the Chronology Sheet of the license application, must I list all of the jobs I have ever had?
    It is not all that important that all jobs are listed, it is important that all time periods be accounted for. Leave no blank time periods.
     
  8. When does health care insurance for me and my family take effect?
    Your insurance for you and your family begins the date of hire.
     
  9. What do I do about orientation if I am currently in a training program and will not complete that training until June 30?
    All incoming house staff are strongly encouraged to attend general orientation. If there are reasons beyond your control, ie. not having completed your program, arrangements will need to be made for “mini’ orientation. This is just upper level residents coming in as PGY II or greater. E-mail the
    House Staff Office with these concerns and you will be notified as to options available.
     
  10. How do I find information of banks and real estate agents?
    We recommend checking with the Winston-Salem, Chamber of Commerce. If there are banks or real estate agents offering special promotions for new house staff, it will be noted on their website.
