Intellectual Property Disclosure Form
For the University inventor or developer, the first step in the commercialization process is to formally disclose your discovery or other intellectual property to the Office of Technology Asset Management. You can do this by downloading the form here.
If you have a biological material to disclose (vector, cell line, antibody, mouse, etc.) please download the biological material disclosure form by clicking here.
This form establishes a formal date of creation or invention for your discovery and is thus a very important document. Although you may download this form and complete it on your computer for your convenience, it is important that we receive one original that has been signed by all inventors, witnessed and dated.
In addition to completing our disclosure forms, it is also important to update your lab notebook to complete your record of invention. Lab notebooks should:
· Be bound, not loose-leaf
· Be signed and dated
· Be in ink, not pencil
· Not have any blank spaces
· Not be modified
· Be written in past tense
· Have abbreviations and terms explained
· Have attachments stapled in
· Not have original attachments removed
· Provide outlines of new experiments
· Provide a record of lab meeting discussions
· Be detailed
· Be tracked, numbered, etc.
· Be saved after completion
