Instructions for Physicians:

We have streamlined our scheduling procedures to make it easier for you to refer your patients for endoscopic procedures such as elective colonoscopy and upper endoscopy. And as always, we are interested in seeing your patients in full consultation for gastroenterological symptoms and diseases.

Simply call the Digestive Health Center at (336) 713-7777 to schedule your patients for these procedures. Our scheduler will ask you about several important medical conditions that will affect our ability to do the procedure safely for your patient.

You may also use our simplified request forms and fax them to our office at (336) 713-7322. Click on the links below to get a copy of the form you need.  The forms can be printed out and completed by hand, or you can fill them in onscreen, print them out, and fax them to us.

Our schedulers will use the information you provide to contact your patients and set up a time for them to come in for consultations and/or procedures, and then we will let your office know when it is scheduled.

Note that we have expanded our faculty and have reduced waiting times for patients who need elective procedures. Feel free to call Dr. Kenneth Koch or one of our on-call gastroenterologists via the Physician Access Line line (for physicians only) if you have any questions about scheduling these services.
