For almost 15 years I have been interested in the pathogenesis and prevention of vascular catheter infections. This has involved obtaining a better understanding of the diagnosis of vascular catheter infections using sonication. Sonication allows the quantitation of microorganisms removed from catheters over a range from 50 to 107 cfu and demonstrated that there is a relationship between number of cfu removed and the risk of bloodstream infection and purulence. The use of sonication in combination with a animal models of catheter-related infection, have made it possible to examine a number of questions in animals that would be quite difficult to evaluate in patients. These include why silicone catheters have a greater risk of infection than other catheter materials, and the relative efficacy and toxicity of various anti-infective coatings on vascular catheters.
I have also been interested for some time in mechanisms of transmission of nosocomial infections. Investigations of this type include the importance of the environment in the transmission of MRSA and acinetobacter, complex IV gadgetry in the transmission of IV infections, and the importance of airborne transmission in nosocomial aspergillus and S.aureus infections.
