Blood Bank Rotation

Rotation Details/Objectives:

Upon completion of this laboratory rotation, the resident will be able to demonstrate competence in the following areas of knowledge and practice:

Blood Components

  1. Understand the principles and techniques of collection and processing of blood including pheresis procedures. Observation of the Red Cross Blood Center provides exposure to a large-scale donor pheresis operation, including autologous and directed donations.
  2. Understand the storage and shipment requirements for blood products, as well as thawing, pooling, and special handling of components (i.e. irradiating, washing, or deglycerolizing).

Immunohematology

Apply the principles and techniques of pretransfusion testing by daily evaluation of patient samples that have grouping discrepancies, unexpected antibodies and positive direct antiglobulin tests; diagnosis of hemolytic disease of the newborn; and evaluation of a patient refractory to platelet transfusions. As residents gain appropriate experience, they will be required to process the interpretive reports explaining the significance of atypical antibodies to the patients' physicians.

Transfusion Practices/Quality Assurance

Evaluate daily the indications and appropriateness for transfusion of all components from the preceding day. Review the blood administration self-assessment manual and observe at least one transfusion each week.

Transfusion Reactions

Evaluate adverse reactions to transfusion, order an appropriate follow-up workup, and make clinical recommedations regarding these reactions and subsequent hemotherapy. Investigate all suspected post-transfusion infectious complications.

Electives

  1. Bone marrow transplantation (BMT): Understand the principles and techniques of bone marrow harvesting, processing, storage, and reinfusion following a rotation in the BMT lab.
  2. HLA system: Learn the special techniques of the HLA system during a rotation in the HLA lab.
  3. Therapeutic apheresis (TA): Learn the various disease states that are treated by TA and distinguish appropriate from inappropriate use of TA procedure.
