In this case, the Dean is just a suggestion or recommendation from the Board. You can have the recommendation forms completed by any two physicians who have supervised or worked with your in recent months.
These fingerprints need to be done by someone at your local police station or sheriff’s office or there could possibly be someone in the Security Office at the institution you are now in that could do them. Your license will be held up if the quality of the prints are not good and get rejected. So do not do them yourself. Digital prints are not accepted.
The appointment letter comes from the House Staff Office. That letter will be included with your application prior to forwarding to the Board for your license to be issued. You do not have to do anything to obtain this letter.
Yes. If your medical school will not complete the form until you have actually graduated, you can go ahead and send in the rest of the application and we can review all but that one form. If this information is all correct, the license will just be pending receipt of that one form.
You must send in a copy of your marriage license with your license application to the House Staff Office in order for the Board to issue your license in your married name. Our office will also use that marriage license to change your name in all the various areas in the hospital.
These forms are not required for incoming residents who are just graduating from medical school. If you have been out of medical school for 6 months or more or graduated from a foreign medical school, you must complete these forms.
It is not all that important that all jobs are listed, it is important that all time periods be accounted for. Leave no blank time periods.
Your insurance for you and your family begins the date of hire.
All incoming house staff are strongly encouraged to attend general orientation. If there are reasons beyond your control, ie. not having completed your program, arrangements will need to be made for “mini’ orientation. This is just upper level residents coming in as PGY II or greater. E-mail the House Staff Office with these concerns and you will be notified as to options available.

We recommend checking with the Winston-Salem, Chamber of Commerce. If there are banks or real estate agents offering special promotions for new house staff, it will be noted on their website.
