All residents coming to Wake Forest University Baptist Medical Center to do a rotation must provide the House Staff Office with the following:
- Written documentation signed by the program director of the residents sponsoring institution stating that their institution is continuing to cover the rotating residents, salary, benefits, and malpractice insurance during their rotation.
- Verification of malpractice insurance, i.e. certificate of insurance.
- A valid N.C. residency training license. A residency training license can be a time consuming process. We suggest that you obtain the application and begin the process as soon as you determine that you will be rotating here. You must allow at least one month for this process. If a rotating resident does not have a residency training license prior to the time of their rotation, they can do no hands on patient care, therefore the rotation would not be beneficial.
You must obtain a Residency Training License prior to beginning your training program. The North Carolina Board of Medical Examiners has made the application available on-line. Please go to :

Complete the parts on-line that you can. Print the documents that need to be printed and mailed. The Board has provided a section "Residency Training License Status", where you may go in after the application has been completed and submitted and check the status of your own application.
Your on-line application will be electronically sent to the House Staff Office at this institution. Your application will remain with the House Staff Office until all paper credentials and documents arrive from the sources in which you have requested information. Once all required documentation is received, the House Staff Office will electronically submit the on-line application to the Board and mail the paper credentials and documents for process and review.

. Please read the instructions for the application very carefully. Anything not completed properly or in full will cause delays in the application process.
Note: If you have already graduated from medical school, your school must complete “Medical School Certification Form B”. Form A is only for those who have not graduated and the Board will not accept the wrong form. Be sure if you have been certified or licensed in any medical field, i.e. EMT, nurse, certified nursing assistant, etc., you must have either the verification of license or certification form in the application completed by that licensing or certifying board. A training license cannot be issued without that documentation. The application must be forwarded to the WFUBMC House Staff Office, Attention: Diane Hartley.
